What Is Mechanical Integrity Under PSM?

Mechanical Integrity is one of the most technically demanding elements of PSM. Section 1910.119(j) requires employers to establish and implement written procedures for maintaining the ongoing integrity of process equipment. The scope is broad: pressure vessels and storage tanks, piping systems, relief and vent systems, emergency shutdown systems, controls including monitoring devices and sensors, and pumps.

The MI program must include written procedures, employee training on maintenance procedures and hazards, regular inspection and testing performed by qualified personnel using recognized and generally accepted good engineering practices (RAGAGEP), and a process to correct deficiencies — either before further use or in a safe and timely manner when the deficiency is found.

Quality assurance is also part of MI. Before new equipment is installed, it must be inspected to confirm it is suitable for the process application. Maintenance materials, spare parts, and equipment must meet the specifications of the original design.

What the Standard Actually Requires

The employer shall establish and implement written procedures to maintain the on-going integrity of process equipment. Inspections and tests shall be performed on process equipment. Inspection and testing procedures shall follow recognized and generally accepted good engineering practices. Consequences of Mechanical Integrity Failures

Equipment failure due to inadequate mechanical integrity is a primary cause of catastrophic releases. The CSB's 2018 report on US refinery incidents found MI failures — inspection gaps, uncorrected corrosion, inadequate QA — were contributing factors in over 60% of reviewed major incidents.

MI violations are among the most common in major PSM inspections and frequently constitute multiple separate citations — one for each element of the program that is deficient.
