What Is Process Hazard Analysis Under PSM?
Process Hazard Analysis is the analytical core of PSM. A PHA is a systematic, organized effort to identify and analyze the significance of potential hazards associated with the processing or handling of highly hazardous chemicals. OSHA requires that PHAs be performed using one or more of the recognized methodologies: What-If, Checklist, What-If/Checklist, HAZOP, Failure Mode and Effect Analysis, Fault Tree Analysis, or an appropriate equivalent methodology.
PHAs are not a one-and-done exercise. OSHA requires that initial PHAs be updated and revalidated on a schedule not to exceed five years. Each revalidation must reflect changes made to the process since the last PHA. All recommendations from a PHA must be resolved in a timely manner and documented — including the rationale when a recommendation is rejected.
The PHA must be performed by a team with expertise in engineering and process operations, and at least one team member must be knowledgeable in the specific PHA methodology used. Critically, workers who currently operate the process being analyzed must be part of the team.

Consequences of Process Hazard Analysis Failures
PHA deficiencies are directly linked to major incidents. The Texas City explosion (2005, 15 killed) involved a process with a PHA that failed to adequately identify the hazards of the blowdown drum. Inadequate PHAs consistently appear in CSB investigation reports because unidentified hazards cannot be controlled.

Penalties for missed revalidations or failure to resolve recommendations are frequently classified as willful. Per-violation penalties can reach $156,259 for egregious willful violations.
